CHANDLER, AZ (CBS5) -

Chandler police are looking for a 61-year-old man who walked away from a care facility late Wednesday afternoon.

A Chandler police spokesman said Robert Douglas Johnson, who has dementia and Alzheimer's disease, walked away from the Autumn Years care facility in the 200 block of North Washington St.

Johnson is described as white, 5'10" tall, 160 pounds with brown hair, hazel eyes and a salt-and-pepper beard.

Anyone with information about Johnson's whereabouts are asked to call the Chandler Police Department or your local law enforcement agency.
